New Patient Evaluation with one of our Physical Therapists.

PATIENT FORMS

Medical services are provided and billed by Southern Tier Physical Therapy of Binghamton. We work with insurance and workers compensation providers to make our services as accessible and affordable as possible. Below are the health plans which we currently accept. With your permission, we work with your insurance provider to detail your unique benefits.

Copays are due on the day of treatment. We accept Personal Checks, Cash, Visa, Mastercard and Discover.

PAYMENT POLICY

What to bring for your first appointment:

Prescription from your referring doctor (MD), Physician Assistant (PA) or Nurse Practitioner (NP). The prescription is an order for the evaluation and treatment for a specific body part or condition. It must include the date and referral signature. Most prescriptions are valid for 30 days.
Photo Identification (i.e. Driver's License)
Insurance Card(s) or Billing Information (i.e. No Fault, Workers' Compensation card, VA card...)
Payment Method if Necessary
Optional: Downloaded & completed forms (see details below)

When to arrive:

Please arrive 15 minutes prior to your scheduled appointment time in order to check-in and complete the required paperwork. For your convenience, you may click the links below to download and complete many of the forms. Please note that there may be additional paperwork to complete upon arrival.

What to wear:

Loose fitting clothes helps us evaluate and manipulate the specific body part to be treated.
