1. Prescription from your referring physician, PA, NP, podiatrist, or dentist (if applicable).

  • A 'prescription' should be an order for Physical Therapy written by your physician to evaluate and treat a specific body part or condition. It must include a current date and physician's signature to be accepted. For most insurances, physical therapy orders are valid for 30 days from the date prescribed.

2. Photo Identification (i.e. Driver's License)

3. Insurance Card(s) or Billing Information (i.e. No Fault, Workers' Compensation, V.A.)

4. Payment Method if Necessary

  • Copays are due on the day of treatment. We accept Personal Checks, Cash, VISA, Mastercard, and Discover. At this time, we do not accept American Express. If the necessary payment is not made the day of treatment, we will bill you for that date of service.

5. Loose Fitting Clothing

  • This will allow for evaluation and manipulation of the injured body part.



What you need to bring to your first appointment:

At this time we do not accept walk-ins. Treatments are by appointment only. If you do not already have an appointment and would like to schedule one, please contact our office at 607-771-8181 and we will be happy to set you up for an evaluation with one of our Physical Therapists.

Please arrive 15 minutes prior to your scheduled appointment time in order to check-in and complete the required paperwork. For your convenience, you may click the links below to download and complete many of the forms. Please note that there may be additional paperwork to complete upon arrival. 

A pediatric intake form should also be completed by a parent/guardian for patients ages 0-10.
